$25 Flu Vaccine. One per flu season (September-April).
$25 Chlamydia Screening. Age 16-24. One per calendar year.

$30 Diabetes HbA1c Test. Age 18-75. Once per year.

$10 Pre and Postpartum Depression Screening. Complete Edinburgh depression screening with care manager. Up to two total (prenatal and postpartum).
$25 TDAP for Pregnant Women. One per pregnancy.
$15 Initiate Substance Use Treatment. Complete one treatment session within 14 days of substance abuse disorder diagnosis.
$30 Substance Use Follow Up Visit. Age 13+. Follow up visit within 7 days after emergency department visit.
$30 Mental Illness Follow Up Visit. Age 6+. Follow up visit after emergency department visit.
$10 HPV Vaccine. One per calendar year.
$10 Rotovirus Vaccine. One per year.
$25 Notification of Pregnancy. One reward per pregnancy.

*To be eligible for this reward, you must notify us you are pregnant prior to having your baby by calling us or submitting a completed Notification of Pregnancy (NOP) form.
Your My Health Pays™ rewards are added to your rewards card after we process the claim for each activity you complete. If you are earning your first reward, your My Health Pays™ rewards card will be mailed to you.

Once you complete a qualifying healthy activity, we are notified, and your card will be mailed to you. Remember to keep your card, as future reward dollars will be added to it for each qualifying healthy activity you complete. If you have questions, please contact us at 1-844-366-2880.
Once you receive your My Health Pays™ rewards card in the mail, please follow the instructions on the activation sticker. You will need to have your card with you to complete activation.
To check your balance, login to your Secure Portal account, or call the number on the back of your card.
Funds expire 90 days after termination of coverage or 365 days after date reward was earned, whichever comes first.
