MTM Health

If you need help getting rides to and from your doctor's appointments, call Member Services at 1-844-366-2880 (for TTY/TDD: call 1-844-804-6086, Relay 711). You will talk to someone from MTM Health*.

They are open Monday through Friday from 8:00 A.M to 6:00 P.M. Call at least 5 business days before your appointment so they have time to arrange a ride for you.

*Please know, MTM Health may have requirements for certification before being able to access this benefit.

Modivcare

Need a ride? We’ve got you covered for trips that are not covered by MTM.*

We can get you to the pharmacy, WIC offices, health district offices, food pantry, DMV, and grocery stores.   

Contact Member Services at 1-844-366-2880 (for TTY/TDD: call 1-844-804-6086, Relay 711) or your Case Manager to schedule a ride.

Make sure to call at least 48 hours in advance for a guaranteed ride. Each member can have up to 5 round trips per year.

Transportation Bus Passes

Transportation bus passes help Members get to places where they can find community support and services. If you need help, we can figure out the best way for you to travel and give you one-day bus passes.
