Application for SilverSummit Community Reinvestment Funding
Thank you for your interest in the SilverSummit Healthplan Community Reinvestment Program.
Before getting started, please review the application instructions provided below for important technical and content requirements. Taking a few minutes to review the instructions in advance will help streamline your submission and avoid delays in processing.
Once you're ready, complete the form with your organization's information and contact details. Then, upload the required supporting documents.
Application Requirements
- Complete the following required documents:
- Face Sheet (PDF) Add the Face Sheet as the first page of the proposal.
- Proposal (Word doc)
- Budget Template (Excel Spreadsheet)
- Combine all documents in a single PDF
- Submit your application
Once your proposal is assembled. Go to the Submit Application section below, complete the required form, and upload your PDF proposal. Click Submit to finalize your application.
A confirmation message will appear when your application is successfully submitted.
Answer all questions using clear, simple language
Make sure your proposal:
- Meets eligibility requirements
- Aligns with priority focus areas
- Focuses on measurable outcomes
- Shows how you will collect and track data
Keep your content clear, concise, and focused
Do not include promotional materials or unneccessary design elements
Maximum 6 pages (not including the Budget Template); Charts, tables, and graphics do not count toward the page limit
1-inch margins
12-point Calibri font; Smaller font allowed for charts, tables, and graphics
Single-spaced text with one blank line between paragraphs
Include a header or footer on every page with the Organization name and a page number
Combine the Face Sheet, Proposal, and Budget Template into one single PDF.
