Housing Supports and Services (HSS)

What are HSS? 

Housing Supports and Services (HSS) are new Medicaid services that help people find and keep safe housing. These supports are for people who are at risk and need help staying healthy and safe. SilverSummit Healthplan, through our Housing Roots program and partner agencies, can help you get and keep housing.  Using HSS is optional. Your regular Medicaid benefits will not change.

Services You May Get 

  1. Specialized Case Management: We talk with you about your needs and goals. Together we make a simple plan to help you find housing. 
  2. Housing Transition Supports: We help you search for a home, fill out forms, talk with landlords, and plan your move. 
  3. Housing-Related Deposit: We may help pay one-time move-in costs, like your deposit, first or last month’s rent (f required) and utility deposits 
  4. Housing Sustainment Services: After you move in, we help you stay housed. We check in, helping with landlord issues, and connect you with budgeting or local help.

Who Can Get Help

You, a family member, or another person in your home may qualify if they:  

  • Have active SilverSummit Medicaid, and 
  • Are homeless or at risk of losing housing, and 
  • Have one or more of health-related needs, such as: 
    • Left a hospital, jail, prison, or treatment center within the last 90 days, or will be released within 30 days 
    • Are pregnant or had a baby in the last 60 days 
    • Have a serious mental illness, emotional disturbance, or substance use disorder 
    • Are at risk of a medical or behavioral health crisis (for example, frequent ER visits) 
    • Are a survivor of domestic violence or human trafficking 

HSS can be provided only to people who have active and valid Medicaid eligibility.  For full rules and definitions, visit the Nevada Medicaid HSS Policy Guide.  

Before You Get Help

We will check if you qualify.  

You may need to share proof of: 

  • Your health needs 
  • Your income 
  • Your housing situation 

If you ask for a deposit help, you must also take part in the other HSS supports.  

You may be considered homeless or at risk of homelessness if: 

  • You do not have a safe place to sleep, or 
  • You live in a shelter or a place not meant for people to live, or 
  • You are leaving jail, prison, or a hospital and have nowhere to go, or 
  • You will lose your housing in the next 30 days, or 
  • You are a child or youth without a regular place to stay.

1. Call Member Services 

  • Phone: 1-844-366-2880  (TTY/TDD: 1-844-804-6086 Relay: 711). 
  • Hours: Monday - Friday, 8 a.m. to 5 p.m. PT.  

Ask for the Housing Supports and Services (HSS) team.  A team member will call you back within 72 business hours. 

2. Use Find Help Online 

  • Scan the QR code or visit the SilverSummit FindHelp page 
  • Enter your zip code 
  • Choose Housing 
  • Pick a provider 
  • Click Next Steps 
  • Fill out the short form and Submit 

Your information goes to a housing provider. They will contact you to complete a short screening. 

  • HSS is optional.  
  • If your request is denied, you can appeal or ask for a fair hearing. 
  • If you change health plans and your new plan offers HSS, your supports may continue.  
  • The housing deposit is one time per lifetime.  
  • Other HSS services may be used again if needed. 
  • If you move to a new health plan, available services may be different.
