Baby's Bounty is expanding the diaper bank to Washoe County. They will give local families access to diapers and wipes thanks to the funding from SilverSummit. Diapering is essential for child health issues, parental postpartum depression reduction, and preventative healthcare.
The funding will help six Clubhouses serve at least 120 SilverSummit youths between the ages of 13 and 18 with workforce development programming. The program will offer first job readiness skills and a plan for pursuing postsecondary education and/or career aspirations.
Client choice market will open in Reno. The expansion will offer clients a grocery store shopping experience and resources center to address individual needs of the clients. Case Workers will provide resources and added services to our members.

In an effort to Improve Maternal, Child and Youth Health, SilverSummit’s funding will support two Community Health Workers, technology, and a pilot of the Southern Nevada Pathways Community HUB to serve Clark County. The program will specifically target pregnant women and work in partnership with SilverSummit Start Smart For Your Baby OB Case Managers.
Funding will support enhancement of CIS’s Integrated Student Supports program and will look to raise graduation rates, leading to a more employable workforce.

Program will be increasing access to CHA's in-clinic food pantries, and increase access to childhood immunizations, the HPV vaccine, the influenza vaccine, and the COVID-19 vaccine.
Healthy Birthday, Inc., will be providing stillbirth prevention educational materials, products and training related to its evidence-based Count the Kicks Program.
SilverSummit’s support will go to the recruitment of a pediatric rheumatologist to the Las Vegas area. The Pediatric Rheumatology Program will address this significant provider gap in the state.
Funding will support the community performing arts program, the HHEP Academy. The goal is to prevent youth from entering or having further involvement in the justice system through performing arts.
The funding supports SilverSummit Justice Involved Members with a complete residential clinical program, including case management, 12-hour monitoring, stabilization, counseling, meals, and housing.
Funding will support the development of a chronic disease management program and provide added support for patients to achieve healthcare goals and receive preventative screenings. The funding will allow for the hiring of one community health worker and one call center staff person.
Hope Means Nevada was a statewide suicide prevention and mental wellness initiative. This campaign targeted SilverSummit members, with a focus on teens and young adults in Clark and Washoe Counties in fall of 2022.
GRIT - Grown Refine Integrate and Thrive aims to help SSHP Members with SUD to build healthy fitness and nutrition habits to assist in sustaining their path toward recovery. This peer-led approach provides access to evidenced-based physical and nutrition education, along with opportunities to build relationships with their peers.
The Life Changes "Healing Home" offers a safe place for SilverSummit Healthplan members who are housing insecure or experiencing homelessness to rest, heal and recover from an illness or injury. The "Healing Home" is offered to SilverSummit members who need housing and wraparound services to safely discharge from the hospital.
SilverSummit MEMBERS WILL RECEIVE: A bed, meals, Home Health Care, BH Therapy, Case Management including Long Term Housing Support.
Nevada Health Centers will be able to expand to Washoe County thanks to the SilverSummit funding, which will improve access to primary care services. Also, support will enable the healthcare workforce's DEI and cultural competency training to be improved.
Funding will support the purchase of a population health tool to improve outcomes, specifically around diabetes and hypertension. Also, to support the purchase of the Azara population health management tool, and the salary of the NVPCA Health Information Manager.
Funding will support scholarship for SilverSummit women members for secondary education, internships, and mentoring opportunities.
The funding will promote the expansion of access to and enrollment in initiatives that strengthen the improvement of the social and structural determinants of health in the targeted communities.
Northern Nevada HOPES will get funding to build a new 43,000 square foot clinic that will provide adult primary care, chronic disease prevention, case management, and an expansion of the mental and behavioral health services.
Hiring a committed Community Health Worker to reach out to Members will help close HEDIS gaps by increasing member doctor visits.
A Telehealth program that uses video teleconferencing software to connect providers will be supported by the funding. Project ECHO Nevada aims to improve the ability of medical and public health practitioners to successfully treat emerging, common, chronic, and complex conditions in populations across Nevada. The funding also supports the creation of two new project ECHOs, one of which focuses on transgender healthcare and the other on diabetes treatment.

The funding is used to provide housing assistance in the form of rent, security deposits, short-term housing, and move-in costs in order to either avoid eviction or reduce the length of time people are homeless.
The new R.E.A.C.H. Community Health Center (RCHC) is to serve and improve the health of the underserved community and vulnerable populations by assuring access to comprehensive, culturally competent, quality primary health care services.
Research Education and Access to Community Health - REACH Las Vegas (reachlv.org)
The funding aids adolescents who have aged out of foster care with wraparound programs and transitional housing. By providing services, it is intended to lower the barriers to the transition from foster care to independence.
The funding will support SilverSummit members who are assigned a hostel bed in their move to a more secure living situation. To help members who have been placed, the Salvation Army will offer 1:1 transitional care management, meals, lodging, transportation support, and move-in furniture.
In an effort to address the national shortage of infectious disease doctors, funding will be used to establish and support an infectious disease fellowship training program that will begin in the summer of 2023.
The funding will go toward transitional housing for adult men that offers evidence-based substance abuse treatment, re-entry assistance, and professional referrals for mental health issues.
Funding will support:
- The expansion of the no-cost community market which aims to reduce the risk of chronic disease and illness that is associated with poor nutrition and food insecurity.
- Expansion the Farmer’s Market Program by providing wraparound services, fresh produce, nutritious groceries, and nutrition education.
- The delivery of healthy snacks and nutrition education materials to children and their families, to reduce the risk of chronic disease and illness associated with poor nutrition.
The funding is for a subscription and implementation of Stanson Health Software. In order to improve the quality of care for all members, the software will link with their practice management system and alert providers during patient care of services relevant to quality and HCC coding.
Funding will support the re-entry program to provide comprehensive assessments, community-based SUD treatment and a wide range of wraparound services for 60 adult men and women diagnosed with Substance Use Disorder.
